Clients in case management get food boxes
People with chronic illness who meet the program criteria in Contra Costa County and who are enrolled in case management services through the County Health Department receive food twice a month at no cost. The clients receive a nutritious box full of canned foods and dry goods, a bag of fresh fruits, vegetables and bread, along with a gallon of milk at each distribution. The Extra Helpings Program clients are referred to the Food Bank by their case managers. This program offers home delivery service to those clients who are medically home bound.
