Stock Donation Form

Thank you for supporting the Food Bank of Contra Costa and Solano through a donation of securities. Please contact your broker to complete your transfer. Most brokerage firms are able to transfer stock electronically, which is the most secure and cost-effective way to process your gift. The following information will assist you and/or your broker in completing a transfer of securities to the Food Bank:

•Transfer stock to Vanguard, account number 28948289

•For electronic stock transfers, the (DTC) number is 0062

•We are a 501(c)3 nonprofit organization, tax identification number(FEIN) 94-2418054

To advise the Food Bank of your stock donation and enable us to issue an appropriate receipt, please complete the following form. Please note: completing this form alone will not transfer stock to the Food Bank; you must contact your broker to complete the transfer.
