Agency Relations Change Request (Internal)

This form is to be used by Agency Relations Program staff to communicate program changes that will affect other departments.  Based on the form content, an email alert will be generated to the appropriate groups (Ops, etc.)

Examples of changes to request include:

• Open to the public hours change
• Date change
• Location change
• If a site is converted to drive-thru
